填報表說明：
1.序號為參賽隊員的序號，每位選手只有一個序號。風度隨隊人員,包括觀摩、旅遊、團隊工作人員的名單報在參賽隊員之後，並在備註欄中註明身份。
2.每個項目填寫一欄，如報三個項目，則填寫3欄，運動員序號不變，分三欄填寫；出生年月填寫4位數字的年份即可，不要填寫周歲數，如1962年5月出生，須填4位數“1962”就可以了，不準填年齡“50”或2位出生年份數“62”。
3.對練項目、對抗性項目必須在備註欄內註明配對人員的序號；報多個相同套路名稱的集體項目必須在備註欄內註明“集①”或“集②”，加以區別。
Indication：
1. Athlete no. is marked at the back of list to identify their participation.
2. Each item please fill one column, if apply three, then fill three column; Date of birth just fill in the year, for e.g. 1962, please don’t fill the age
3. Pair event and confrontation event must mark the parter’s item no. in the remark column. If register more than one item, it should be marked group (1), group (2) to distinguish it.
